URGENT: SLIM XTREME™ RECALL

This is to inform you of a voluntary product recall involving: ANABOLIC XTREME, SLIM XTREME, Dietary Supplements. The product is distributed as a box, containing forty-five (45) capsules per package. The label on the packaging lists the company name Nutrition Distribution,
Tempe Arizona. In addition, sample packets containing 3 capsules per packet were given as samples and are labeled SLIM XTREME. All lot codes are covered in this recall.

This voluntary recall has been initiated due to recent allegations that an ingredient contained in SLIM XTREME may not comply with all provisions of the Federal Food, Drug, and Cosmetic Act. At this time, Nutrition Distribution is aware of no serious adverse events resulting from consumers ingesting this product. With an abundance of caution, we are voluntarily recalling all lots of SLIM XTREME
from the market as a precautionary measure.

Please examine your stocks immediately to determine if you have any SLIM XTREME product on hand. If so, discontinue sale of the Slim Xtreme products and promptly return them to our headquarters location in Tempe, Arizona. Please obtain an RMA number from us to ensure a full refund/credit and label all returned containers as SLIM XTREME Returns RMA# ___________.

If you have distributed any of these products, please immediately contact your accounts, advise them of the recall situation, and request they return their outstanding recalled stocks to you for credit. Once received, please return these stocks to Nutrition Distribution for full credit as indicated above. You may forward a copy of this letter if you wish however you must add specific directions to return the product to you for credit according to your policies. Please document your efforts carefully. This would include having your accounts complete the information on the RETURN FAX regarding the amounts on hand that they are returning. Maintain this tracking for any subsequent returns and to allow an audit of the recall effectiveness.
